We deliver a range of support services to Commonwealth Home Care Package recipients, as well as to privately funded clients.

Start your journey today with My Aged Care
If you are new to aged care services, then the Commonwealth Government’s My Aged Care phone line 1800 200 422 and website is your first point of call. You may be eligible for more services than you think!
My Aged Care provides information on the types of aged care services available, including funded Home Care package options, eligibility requirements, costs and assessment requirements.
If you believe that you are eligible for Commonwealth funding, then the first step is to register with My Aged Care. An assessment will then be undertaken to determine what level of Home Care Package funding you may be eligible for. This will include a face-to-face assessment of your care needs, as well as a financial assessment.
The assessment process will differ dependent on your level of need:
Low level home support
Commonwealth Home Support Programme (entry level care)
including help with housework, personal care, meals and food preparation, transport,
shopping, allied health, social support and planned respite.
Assessment by the Regional Assessment Service (RAS).
More complex care needs
Home Care Package Program (more complex coordinated care, four levels)
including personal care, support services and nursing, allied health and clinical services.
Requires comprehensive assessment with an Aged Care Assessment Team (ACAT).

Once you are assigned a home care package, then we can provide direct services to you, as your primary service provider or as part of an integrated care team.
You will just need to provide us with your MyAgedCare referral code and the results of your financial assessment. We can then detail our fees and charges, and work with you to develop a tailored, individualised care plan that helps you to reach your goals.
Transfer your existing Home Care Package to Assured Home Care
If you have an existing home care package in place and wish to appoint Assured Home Care as your new service provider, you will just need to:
- Notify your current provider and agree a cessation date. This may be a mutually agreed date, or may be subject to exit conditions specified in your Service Agreement with your current provider.
- Advise MyAgedCare that you are changing providers and obtain a new referral code
- Provide us with your new MyAgedCare referral code and let us know the date from which you would like to commence services We can assist transition on your agreed cessation date, to ensure service continuation. We will work with you to develop a tailored, individualised care plan and budget that best supports your needs. We will then detail our fees and charges.
